With this form you define a Custom form and you indicate the Customization groups to which it applies; see Custom form.

Header
- Custom form: Identifier of the custom form, unique in the application.
- Name: Name of the custom form. Summarize the characteristics of this form.
Form
- Module: Selecting this allows you to reduce the list of functions offered. In any event, it will be filled in according to the function.
- Function: The function to which the form belongs.
- Form: The form to be customized from among those of the selected function.
- Filter: To be selected so that the advanced filter tab can be customized. The filter must be public.
- Highlighting: To be filled in so that special highlighting (for a result form) will be applied. The highlighting must be public.
- Edit the form: This button causes the form to be displayed in customization mode.
- Display: Is used to define the custom form display type (grid mode or otherwise). "Apply default mode" is selected as default.
History
- Created by: Cost of the user who created the custom form.
- Version: A free field for indicating that changes have been made and their scope.

Source: Indicates from where the custom form comes,
- "Customized" if you have created it,
- "Standard" if it is bundled with CARL Source,
- "Integration" if it was made by CARL Source for your needs.
- Changed on: Date of the last change to the custom form.
Access
- Confidentiality: Criterion for limiting access to the custom form according to the data access group and the user.
The confidentiality must be defined as an access criterion of the "Custom form" entity. Confidentiality is a configurable values list.
- Public: If this box is checked, the form will be applied to all customization users and groups.
Uncheck in order to be able to restrict the list of customization groups in question.
Customization groups
This list contains the customization groups to which this custom form applies.
In order to be able to add and delete customization groups, uncheck the "Public" box.
Application values
This list contains the values of the customization attribute (defined on the object in the dictionary) to which this custom form applies.
- Code: Customization attribute.
- Name: Attribute name.
- Creation menu: Allows you to define a new creation menu entry for the entity directly with the attribute value of the line and the custom form.
- Menu name: Name that will appear in the customized creation menu entry
The customization form name is displayed in the menu pop-up. Its meaning must be clear to the user of the application.
The presence of a customized creation menu can be defined with its name to directly access the custom form in creation mode, with the information attribute value automatically filled in the form function.
There are 2 types of menu:
- In list: The menu entries are added to a list, under the creation menu.
- Via a pop-up: A pop-up window opens in the case of creation menus associated with custom forms, where the user can choose whether to create the simple menu or a custom form and its customization value if there are several of them.
The menu type chosen can be found in the system configuration, HMI category, "customFormAttributeMenuType" parameter.
In the case of a list menu or a pop-up menu, the names will not have the same form. In a list, it is more appropriate to fill in an action, whereas in a pop-up, it is more appropriate to fill in the component to be created.
Some of the application's creation menus are already customized and do not allow you to use the "In list" option. In this case, calling the entity creation will automatically propose the choice through the pop up selection menu.
The following entities are involved: locations, contracts, purchase/transfer requests.
Example:
- By selecting the VEHICLE value for the "Equipment type" attribute of the MATERIAL object for the customized asset detail form, and by filling in a menu with the name "Vehicle creation" name, a menu with this name will be included in the list of the Asset creation menu. When this creation menu is activated, the custom form of the details of an Asset for the equipment type is displayed by setting the equipment type value to "VEHICLE".
By adding a workflow with an alternative branch depending on an attribute value and configuring an equipment type with default characteristics, the user can create a highly customized component through configuration.
