• Date (required): Date when the event took place. It is necessarily prior to the current date.
• Asset/Structure point (required): Identifier of the element in question; required.
• Name: Name of the element in question.
• Symptom (required) and Name: Type of event likely to trigger implementation of a Work order plan. If necessary, complete the list of symptoms.
• Comment: More precise description of the event.
