When the add icon 
is clicked on, the index detail form is displayed in creation mode.
- Revision index (required): Identifier of the revision index, unique in the application.
- Name: Name of the index.
- Interval (required): Interval for the index - Monthly, Bimonthly, Quarterly, Twice-yearly, Yearly.
- Values: In the list of values, click on the add icon to add a new line in creation mode.
The number of columns accessible depends on the interval selected.
Example:
For a quarterly index, only four values can be entered during the year. The columns that can then be accessed are: January, April, July and October.
